Magnetic resonance image of the brain T2 (A) and FLAIR (B) sequences showing hyperintense punctate lesions, less than 5 mm in size, with subcortical localization in the predominantly right frontal region, probably secondary to previous vascular damage. Magnetic resonance angiography sequences (C) and black-blood sequences (D, E) did not show vasculitis. Molecular imaging obtained by means of 18-FDG-PET showed an intense frontal hypermetabolism in both hemispheres (F), and occipital hypometabolism (G). The Scenium method confirms the pattern of hypermetabolism in the lateral frontal cortex, bilaterally, with a predominance in the right hemisphere (H). The abnormal anteroposterior gradient is clear in the axial projection of the 18F-FDG-PET/CT, with severe frontal hypermetabolism and parietal hypometabolism (I). To improve the interpretation of these results, comparative images are provided, acquired from a woman of a similar age, without neurological or psychiatric disorders (J-M).
