The argument for studies needing to be finerenone vs “active comparator” falls flat when the so-called active comparator has not been shown to be better than placebo in these populations. The argument makes more sense for ASI vs nsMRA than it does for nsMRA vs sMRA. 5.5K views · 21 likes · 6 reposts · 2 replies Open on X →
@AndrewJSauer @escardio Because ESC interprets MR blockade as a class effect. FINEARTS-HF provides the strongest evidence, while steroidal MRA support comes mainly from TOPCAT/meta-analysis. The Class I is therefore partly extrapolated. https://t.co/Vdn9547iVU 1.3K views · 8 likes · 0 reposts · 0 replies Open on X →
@AndrewJSauer @escardio It is due to the meta-analysis of MRAs presented in London last year, and does not take into account SPIRIT (which is not published yet; we all know its flaws, but it is still evidence) https://t.co/KQyvPz4c47 2.3K views · 6 likes · 0 reposts · 1 replies Open on X →
@AndrewJSauer @escardio Had TOPCAT utilized same PEP as FINEARTS-HF, it would have made the cut. There is no material difference in CV or Kidney outcomes with either Rx. Agree neither trial warrants Class 1 rec for HFpEF unless avoiding HFH is a top priority equal to mortality. 6.9K views · 22 likes · 1 reposts · 1 replies Open on X →
I’m sorry but how does @escardio give MRA (without differentiating sMRA from nsMRA) a Class I for HFpEF? Where is the evidence for steroidal MRA? And even if we base on nsMRA, we have one study (FINEARTS-HF) and we somehow end up with a Class I? Make it make sense. 21.5K views · 131 likes · 15 reposts · 14 replies Open on X →

It was a privilege to welcome Dr. Clyde Yancy to Kansas City for a city-wide discussion on contemporary medical therapy for heart failure and the evolving challenge of sudden cardiac death risk. Few leaders have had as much influence across the modern eras of heart failure care 7.6K views · 72 likes · 13 reposts · 4 replies Open on X →
