Later this month, I’ll have the amazing opportunity to meet real-food champion Mike Tyson in Washington, DC…
I’m proud to support real food.
I’m proud to support ketogenic diets and metabolic health.
And I’m honored to represent the low-carb community at an upcoming Summit in

@DoctorTro The reduction of LDL is not indicated, and neither is the use of statins. Period.

10/ Before employers and patients are directed toward the most expensive option, we should determine whether an inexpensive generic could achieve substantial LDL-C lowering in the appropriate phenotype.
Better biology. Better targeting. Potentially far lower cost.

9/ This does NOT prove ezetimibe is equivalent to PCSK9...
It was a retrospective, hypothesis-generating case series of 14 patients...
But a median 53% LDL-C reduction is large enough to demand prospective study.

7/ This is precision medicine:
Don’t just ask, “How high is the LDL?”
Ask, “Why is the LDL elevated in this patient?”
Understanding the mechanism may help identify a treatment that is more targeted, accessible, and affordable.

6) Lipidologists unfamiliar with ketogenic diets previously said this is due to genetic defects in ABCG5/8
We specifically tested ASCVD in multi-gene panels and found to relation to FH genes or sitosterolemia genes.
Furthermore cholesterol balance testing did not predict this

8/ The economic question is unavoidable.
Generic ezetimibe can cost roughly $10/month.
PCSK9-targeting therapies can cost employers and health plans hundreds of dollars every month... often around $700... before administration and other costs.

5/ Ezetimibe blocks intestinal cholesterol absorption through NPC1L1.
Our paper proposes that chronically low insulin may alter cholesterol esterification, absorption, recycling, and transport through unknown and novel pathways.

4/ The magnitude matters.
In broader clinical trials, ezetimibe monotherapy typically lowers LDL by 15%.
The response in this cohort was 3 times what is generally expected...
Why might these patients respond differently?

3/ Four classes of drugs have been shown to DECREASE plaque, decrease CV death and also lower cholesterol (PCKS9, statins, ezetimibe, bempedoic acid).
Other drugs and supplements have been shown to reduce plaque outside of cholesterol too (EPA, colchicine, nattokinase and

2/ We studied 14 patients who developed substantial LDL-C elevations while following a ketogenic diet.
After treatment with ezetimibe:
• Median LDL-C fell >53%
• Total cholesterol fell nearly 39%
• Triglycerides improved
• HDL-C remained stable https://t.co/EiciKELnV8

1/ NEW STUDY: https://t.co/Vipdyqvl5b
If a ~$10 generic drug reduced LDL by 53% on a ketogenic diet why should we use statins or injection antibodies?
Why are cardiologists pushing drugs costing ~$700/month?
A thread 🧵

🚨NEW PAPER🚨 https://t.co/L8LRquSKXZ
@BioMedCentral: Lipids in Health and Disease.
Ezetimibe ⬇️ LDL-C by a median of 53% in patients with keto-induced hypercholesterolemia.
That is ~3x! what you would expect in conventional hypercholesterolemia (~18%).
