My cardiac event came with a plan. Medication, a follow up, and a number to keep under.
Every part of it was about managing what had already happened.
Nobody asked why a fifty two year old with normal cholesterol had a heart attack in the first place. That question was not on h

I had a cardiac event at 52. My LDL was 110. Normal by every guideline in the world.
My doctor never tested my Lp(a). He never measured my ApoB. He told me my cholesterol was fine and handed me a bag of statins.
Most people think LDL, ApoB, and Lp(a) are three separate things.

I keep hearing the same thing since HORIZON failed.
“There are four more trials. Different molecules. Different approaches. More shots on goal.”

@markkaplan20 There are publications showing that increased saturated fat consumption lowers Lp(a).
Might as well eat butter.

8,323 patients. A decade of development. Billions invested. A drug that did exactly what it was designed to do.
And the question the industry will spend years avoiding is the simplest one of all.
If lowering the particle does not change the outcome, was the particle ever the

My Lp(a) is 175. I take this personally.
I was waiting for this trial like millions of others. Hoping that a drug could reduce the genetic risk I was born with. The one thing I cannot change with diet, exercise, or metabolic optimization.
The drug worked on the number. It did

This is not the first time this has happened.
CETP inhibitors lowered LDL dramatically. Failed to reduce events. Torcetrapib. Dalcetrapib. Evacetrapib. Billions spent. Four trials. No benefit.
PCSK9 inhibitors lowered LDL to levels never seen before in human history. The https:

Recent research has shown that the primary mechanism by which Lp(a) causes vascular damage is superoxide production and eNOS uncoupling.
Lp(a) generates reactive oxygen species that break the eNOS enzyme. When eNOS uncouples, it stops making nitric oxide and starts making more h

Here is the question that the entire pharmaceutical industry does not want to sit with tonight.
If Lp(a) causes cardiovascular disease, and you remove it from the bloodstream, why did the patients not get better?
There are only two possible answers.
One: Lp(a) is associated ht

First, let me be clear about what this drug was. Because precision matters.
Pelacarsen is not a statin. It is not a PCSK9 inhibitor. It was not designed to lower LDL or ApoB. It was designed from the ground up, over more than 20 years of research, to specifically and exclusively

My Lp(a) is 175. I have been waiting for this trial for years.
Yesterday, Novartis released the results of HORIZON. The biggest Lp(a) outcomes trial in history. 8,323 patients. A drug built from the ground up to do one thing: lower Lp(a).
The drug worked. The cardiovascular ht

@markkaplan20 EXACTLY!!!!
My Dr states I’m preDiabetic and hyperlipidemic in my chart. I’m prob the healthiest patient he has. My fasting insulin is 2.3. A1c is 5.6. Fasting glucose is 74. Trig/HDL is .6 with a 300 LDL (LMHR). My HOMA-IR is .42.
I’m amazingly healthy…that’s w

My HbA1c is rising. My insulin is falling. My fructosamine is at the low end of normal. My trig to HDL ratio is 0.81. My testosterone is 795. I am drug free and performing better at 58 than I was at 40.
The system looks at my HbA1c and my LDL and sees a patient to medicate.

If you eat low carb and your HbA1c is creeping up while your metabolic markers are improving, here is what to ask.
Get a fructosamine test. It costs less than $30. It will tell you whether your glucose control is actually worsening or whether your red blood cells are just living

My LDL is 171. Flagged high in red on every lab printout.
But my triglycerides are 63 and my HDL is 78. That means my trig to HDL ratio is 0.81. That ratio is one of the strongest predictors of small dense LDL particles and cardiovascular risk. Mine is exceptional.
An LDL of

Now look at the insulin.
When I started this journey after my cardiac event at 52, my fasting insulin was above 6. Not terrible by lab standards. But not optimal. Insulin resistance was building.
Over the last four years it fell steadily. It is now 3.7. Optimal is 2 to 5. I am

There is a test that settles this. It is called fructosamine.
Fructosamine measures glycation over 2 to 3 weeks instead of 3 to 4 months. It does not care how long your red blood cells live. It eliminates the variable entirely.
My fructosamine is 216. The reference range is 205

When your blood sugar is consistently low, your red blood cells take less glycation damage. Less damage means they live longer. Instead of 120 days, they may live 140 to 150 days.
A longer-lived cell collects more glycation simply by being alive longer. Not because blood sugar

HbA1c measures how much sugar has stuck to your red blood cells over their lifespan. The higher your blood sugar, the more glycation. The more glycation, the higher the HbA1c.
A normal red blood cell lives about 120 days. So HbA1c is a 3 to 4 month average of glycation damage.

My HbA1c has been rising for two years. From 4.8 to 5.4.
In the same two years my fasting insulin fell from over 6 to 3.7. My metabolic health has never been better by every other marker I track.
