Whenever crossover exceeds 30% (as in this case), OS becomes very challenging to demonstrate. Which usually implies that the sequence (using the new regimen in 1L vs 2L) does not impact long term outcomes.
In these cases, using the most tolerated regimen upfront is reasonable.

Multiple strategies are being pursued to optimize ADCs safety. Dose optimization is currently key to balance risks/benefits of ADCs. In the future, innovative constructs, diagnostic tools, improved conjugation & patient selection (eg. PGx) are expected to lead to improved safety.
