@AnitaE91496 This looks a lot like a long-standing fungal infection (athlete’s foot + possible nail fungus). After 10 years he really needs a dermatologist to confirm it and get proper treatment OTC creams usually aren’t enough at this point. Keep the area clean and very dry in the meantime.
