@sfchronicle Six providers were waiting on nearly $5.6 million as of July. Scripps Health in San Diego is owed $3 million. A nonprofit ambulance service in rural Pennsylvania is owed about $197,000, which is 13 percent of its annual revenue, and has kept answering the calls.
